The following is offered to assist those groups
and individuals wishing to establish a scholarship with the Texas Dietetic Association Foundation (TDAF). TDAF has been designated as a 501 (c) 3 organization by the Internal Revenue Service.
All contributions are tax deductible.
There are 3 steps to establishing a scholarship:
1. Download the TDA Foundation Scholarship Establishment Form

file (For Word Pad, Microsoft Works or Publisher. Please right click to save to your hard drive; otherwise the form may open in your browser.)
2. Complete form and submit to:
