You can now fill out theTDA Foundation Scholarship Application Form directly in your browser or Adobe Acrobat software. After completing the form, print it, sign it, and mail an original and four copies (refer to Application Instructions).

If you do not already have Adobe Acrobat or Acrobat Reader version 6.0 or newer, you will need to download the free Adobe Acrobat Reader to open these forms within your browser. To get it, click here.

Downloading Instructions

To save:

• At the top left, click computer disc icon(far right).
• A Save A Copy menu pops up. Then Save to location you will remember, i.e. desktop, drive E.

Difficulties

If you find you encounter problems downloading the documents try the following options.

1. Ensure that you have the most recent edition of Adobe® Reader®. You can download it on the Adobe site.
2. Restart your computer and then try to download the document again.
